The shot was taken during the European leg of the Lateralus tour, so it's pretty old and pre-dates Maynard "setting up shop" against smoking.
My guess is that he's since given up, and that cigarette smoke perhaps evokes cravings he'd rather avoid. It's probably doing his voice a few favours as well.

The 'cigarette' also looks to be longer than even a 100....highly unlikely it is a cigarette although the shading does resemble one...looks kinda like a marker or something

maybe maynard asks because all those smokes lit 'dry' his throat during the show....
..I also heard nicotine enhances/engages creativity , possibly?..... I know when I paint I like to step back and take a minute to reflect on the art/moment. like a cigarette after fucking.
